What is best way to terminate? Phone call, letter, email? How much notice is fair if none written in contract?

I think a letter with a verbal communication face to face as well.

I think 2-4 weeks notice is plenty but if it's destructive or disrespectful behaviour than immediate notice would not be unreasonable.

I would type a letter and then also discuss it in person at pick up. I think 2 weeks is sufficient as long as no one will be at risk if safety is an issue. I have also given the option of returning their deposit in lieu of notice.

If you want to be professional then in person, along with a letter. Two weeks notice, unless it is due to violent/aggressive/abusive behaviour then it could mean immediate termination with no notice given.
