Ditto ... mine too cause it is just easier to have one form receipt that I just change the amounts and names on and print out ... so even if they left mid year that part of their receipt says ".... I hereby declare that as a childcare provider I have received the amount of $ XXXX.XX from ___________ for the childcare services rendered during the tax year of 2012 for the child (ren) named, ____________."

This is also what I do. It keeps it simple and includes the whole tax year and any fees I may have received. Even if a parent started half way through the year - it spells out the fact that this is XX$ that I received for the whole tax period and that there isn't another receipt missing somewhere.
