That would be me.

I don't use a form. And the reason I don't use a form is because every reason for termination is different. I type up a letter that is VERY short and to the point. My thought is most parents KNOW why they are being terminated. It doesn't usually just come out of left field.

So, tell me WHY you are terminating.
