    Health info

    I know some of you make index cards to bring with you to the park with dck most relevant info. I currently bring the health forms and a copy of their emergency transport permission form. I want to lighten/simplify things.

    What do you include on your index cards? Do you include a pic of the child and if so, do parents have to complete a picture permission form for this even if it will only be used internally?

    I have 2 pages of emergency forms with contacts, OHIP #, health history and consent from the parents for medical professionals to start lifesaving if necessary and it states they will cover any ambulance cost. I carry the forms everywhere with me in a booklet so that if I ever have to call 911 these forms will be handed to the paramedics as initial consent until parents meet them at the hospital. I don't have a picture on the forms but that is a good point.
